Job Overview:

As the Registered Nurse Care Manager, you would be one of the most important professionals in the client’s life. You will be responsible for providing support to clients enabling them to remain in their home, living as independently as possible. You will be supporting clients with various levels of needs for a safe, happy, and healthy environment. You will assist clients with their daily living activities and personal care. Client support includes, but is not limited to assisting with grooming, bathing, incontinence care, meal preparation, feeding, engaging in mentally stimulating activities, homemaking, companionship, medication reminders, and transportation when needed. The Registered Nurse Care Manager will be in charge of completing in-home assessments and coordination of care.

Job Responsibilities:

  • Conducts clinical direct care for clients
  • Evaluates client care needs, conditions, and assists in developing nursing care plans for individual clients to include rehabilitative and restorative activities
  • Provides instruction in self-help, notifying medical doctor of client choice, and family regarding change in condition of clients
  • Regularly monitors individualized client cases by evaluating treatments and responses
  • Intervenes on care plans and provides personal support as needed
  • Recommends new services and entitlements
  • Perform blood draws and prescription medicine set-ups
  • Conducts meetings and orientations with new clients and their families
  • Determines client requirements by analyzing client’s initial health assessment
  • Develops a care plan which establishes goals based on client’s diagnosis, and incorporates therapeutic, preventative, and rehabilitative nursing actions
  • Establishes schedules and routines
  • Makes necessary revisions as client status and needs change
  • Coordinates, and oversees medical and health care services.
  • Oversees caregivers and assists with delegations and competencies of caregivers
  • Reports concerns to leadership if presented by caregivers or clients
  • Maintains client records by reviewing case notes, logging events and progress
  • Communicates client progress by conducting weekly/bi-weekly/as needed meetings and/or evaluations and disseminates results and obstacles to the leadership team, applicable care givers, case managers, clients, and/or family members
  • Prepares reports by collecting, analyzing, and summarizing treatment, trends, and results
  • Participates in educational opportunities, reading professional publications, maintaining personal networks and participates in professional organizations to continue education and knowledge of the position

Job Qualifications:

  • Active RN license recognized by the Iowa Board of Nursing
  • Must have a valid driver’s license, reliable transportation, and proof of auto insurance
  • Must be able to pass a background check, motor vehicle record check, credential checks, be healthy enough to pass a physical exam and perform all duties and responsibilities
  • Must have exceptionally strong interpersonal, verbal, and written skills
  • Ability to prioritize responsibilities and adapt to changes in daily tasks when necessary
  • Must demonstrate excellent record keeping and documentation skills
  • Must be flexible and accommodating to go above and beyond the basic expectations when required
  • Ability to work varied hours and days to oversee clinical nursing client needs
  • Must work well independently, work with minimal supervision and maintain confidentiality
  • A strong customer service aptitude with high attention to detail is required
  • Must be an active listener and professional in appearance and demeanor
  • Need to demonstrate integrity with patience and compassion
  • Must meet all of the current caregiver responsibilities as well as nursing responsibilities.
